Job summary

Moments Hospice is seeking an RN Case Manager to coordinate and deliver compassionate in-home care, prioritizing patient comfort during final days and weeks. You will work closely with the owners and clinical team to foster growth and clinical advancement.

Responsibilities include initial assessments, care plan refinement, hands-on nursing, patient/family education, and active participation in interdisciplinary team meetings. A company car with fuel and insurance is included.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
