Job summary
Golden Heart Companions is seeking a compassionate Registered Nurse (RN) Case Manager to deliver skilled nursing care and coordinate home-based services for patients in Indianapolis, IN. You will assess needs, develop individualized care plans, and collaborate with physicians, caregivers, and an interdisciplinary team to improve outcomes.
The role requires an active Indiana RN license, CPR certification, and at least two years of nursing experience.
Qualifications
- Graduate of an accredited nursing program.
- Active RN license in the State of Indiana.
- Current CPR certification.
- Minimum of two years of nursing experience.
- Valid driver’s license and transportation.
- Ability to pass a criminal background check.
Responsibilities
- Conduct comprehensive nursing assessments and reassessments.
- Perform skilled nursing visits and complete required assessments.
- Develop, implement, and revise individualized Plans of Care (POC).
- Coordinate services and monitor patient health status.
- Provide medication management and patient/caregiver education.
- Collaborate with physicians, therapists, caregivers, and community resources.
- Supervise Home Health Aides, LPNs, and other staff; support orientation and training.
- Complete timely clinical documentation including visit notes and progress reports.
