Job summary

Care Plus Homehealth Care is seeking a Registered Nurse Case Manager for Home Health in Indianapolis, IN. You will coordinate high-quality patient care, collaborate with interdisciplinary teams, and support patients and families throughout the care journey.

This full-time role requires an Indiana RN license, 1–2 years of nursing experience (home health preferred), and knowledge of OASIS. You will perform assessments, develop plans of care, educate clients, participate in case conferences, and

Qualifications

• Current Registered Nurse (RN) license in Indiana.
• 1–2 years nursing experience (home health or acute care preferred).
• Knowledge of OASIS documentation and Medicare home health regulations preferred.
• Excellent assessment, communication, and organizational skills.

Responsibilities

• Perform comprehensive client assessments and complete all required OASIS.
• Develop, implement, and manage individualized plans of care with physicians and the interdisciplinary team.
• Provide skilled nursing care and serve as primary case manager, coordinating services.
• Educate clients and caregivers on health conditions, safety, and self-care.
• Ensure accurate, timely documentation and Medicare/state/agency compliance.
• Participate in case conferences and quality initiatives.
• Provide on-call coverage, including evenings and weekends.
