Job summary

Pediatric Nursing Specialists of Indiana, a Home Care Network agency, is seeking an experienced RN Case Manager (PRN/As Needed) to coordinate patient care in the greater Indianapolis area. You will perform assessments, discharge planning, and manage a multidisciplinary team in a home health setting.

Flexible, customizable schedules with no on-call requirements, travel up to two hours as needed, and competitive hourly pay between $35 and $40.

Qualifications

• Minimum 1 year of case management experience in home health.
• Experience with OASIS and NON-OASIS assessments.
• Active RN license in Indiana.
• 1+ year in home health or related care; wound care and labs a plus.
• Willingness to travel across counties and serve all ages.

Responsibilities

• RN performs client assessments and directs, plans, and initiates actions in home care.
• RN manages a team of RN, LPN, HHA, and Personal Care Aides.
• RN documents and communicates patient status changes to the care team.
• RN promotes patient safety and a safe environment for clients and coworkers.
• Willing to travel surrounding counties.
