Job summary

Care Plus Homehealth Care is seeking a Registered Nurse Case Manager to coordinate patient care in the home health setting. You will lead assessments, develop and manage care plans, coordinate with physicians and the interdisciplinary team, and educate patients and caregivers.

The role requires an Indiana RN license, 1–2 years of nursing experience, OASIS knowledge, strong assessment and communication skills, and willingness to travel within the service area.

Qualifications

• Current RN license in Indiana.
• 1–2 years nursing experience (home health or acute care preferred).
• Knowledge of OASIS documentation and Medicare home health regulations preferred.
• Strong clinical assessment, critical thinking, and problem-solving skills.
• Excellent organizational and communication skills.

Responsibilities

• Perform comprehensive client assessments and complete all required OASIS.
• Develop, implement, and manage individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Provide skilled nursing care and serve as the primary case manager, coordinating services, monitoring patient progress, and communicating changes promptly.
• Educate clients and caregivers on health conditions, safety, and self-care.
• Ensure accurate, timely documentation and compliance with Medicare, state, and agency requirements.
• Participate in case conferences and quality initiatives.
• Provide on-call coverage, including evenings and weekends, as assigned.
