Help at Home is seeking an RN Case Manager in Indiana to support LaFayette and Logansport regions, with regular travel across assigned territory. The role includes creating and revising care plans, coordinating services, and collaborating with patients and families to meet nursing needs.

Qualified candidates have an Indiana RN license, at least 1 year of clinical nursing experience, and experience with medically fragile clients. A sign-on bonus and comprehensive benefits are offered to new hires.

Qualifications

• Graduate of an accredited nursing school.
• One year of clinical nursing experience.
• Active Indiana RN license.
• Experience with medically fragile clients addressing medical, social and behavioral components of care.

Responsabilités

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and reevaluates nursing needs.
• Initiates the plan of care and necessary revisions.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences.
• Coordinates services.
• Informs personnel of changes in the condition and needs of the patient.
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Processes orders and notifies physician of patient needs and changes in condition.
• Completes certification/recertification orders and discharge summaries.
• Determines the amount and type of nursing needed by each patient.
• Supervises and teaches other nursing personnel.
• Gives total patient care as needed; takes on-call duty nights, weekends and holidays.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• Appropriately utilizes ICD-10 codes.
