Job summary

Independence Home Health LLC is hiring an RN Case Manager to join our compassionate team delivering in-home care. You will assess patients, create individualized plans, coordinate with physicians and families, and supervise LPNs, HHAs, and CNAs to ensure safe, high-quality outcomes.

Requirements include an Indiana RN license, CPR certification, and at least 2 years of nursing experience with home health preferred. We offer strong benefits and opportunities for growth.

Qualifications

• Graduate of an accredited nursing program.
• Active Indiana RN license or a qualifying multistate license recognized by Indiana.
• Minimum of 2 years of nursing experience; home health experience preferred.
• Current CPR certification.
• Strong communication, leadership, organizational, and clinical skills.
• A commitment to providing exceptional patient-centered care.

Responsibilities

• Conduct patient assessments and develop individualized plans of care.
• Coordinate care with physicians, patients, families, and other healthcare professionals.
• Educate patients and caregivers on care plans, treatments, and ongoing health needs.
• Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
• Review and update plans of care as needed.
• Maintain accurate, timely, and complete clinical documentation.
• Participate in case conferences and agency quality improvement initiatives.
• Help ensure patients receive compassionate, safe, and effective care in their homes.
