Job summary

Interim HealthCare of the Twin Cities is seeking a Home Care Registered Nurse (RN) Case Manager to visit patients in their homes, performing head‑to‑toe assessments, coordinating care, managing medications, and providing skilled wound and infection prevention care as needed.

Qualifications

• Graduate of an accredited school of nursing and has a current license as a Registered Nurse in MN.
• Able to travel within a geographic area serviced by the office. Requires reliable transportation, valid driver's license, and proof of auto insurance.
• CPR/BLS Certification.
• Two years of current nursing experience preferred.
• One year case management experience preferred.
• Community health and a wide range of Medical-Surgical experience preferred.

Responsibilities

• Perform assessments and procedures with nursing judgment.
• Visit in-home clients to evaluate care quality and review clinical notes.
• Participate in ongoing interdisciplinary patient assessments.
• Provide services ordered by the physician per plan of care.
• Counsel and educate patient, client, and family on healthcare needs.
• Complete timely and accurate clinical notes and progress.

Job description

The Home Care Registered Nurse (RN) Case Manager enjoys independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The RN Case Manager would provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management,ostomy education, infection prevention, and home safety.

• Annual Salary Range: $89,419 - $106,620

Hours

Full-time | Days | Monday - Friday | Holiday rotation (work approximately two holidays per year) | Weekend rotation may apply dependent upon staffing levels.

Territory

This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position of where the patients live would be the southeast section of the Twin Cities Metro area.

A few must-haves for this position:

Some functions of what you would do as a RN Case Manager:

• Performassessments and procedures, which requireknowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
• Visit in home clients to evaluate the quality of care being provided and review clinical notes.
• Participate in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Counsels and educates the patient/client and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing the patient’s/client’s progress.
• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
