Interim HealthCare of the Twin Cities is seeking an Admissions Nurse (Start of Care SOC Visit RN) to support home care patients in the Twin Cities metro area. You will conduct admissions visits and provide direct patient care in patients0s homes, under supervision of nursing leadership.
The role requires an active Minnesota RN license and at least two years of RN experience; home care/hospice exposure is preferred. You will travel within the service area and participate in a holiday rotation.
Enjoy independence, autonomy and flexibility in the field to care for patients one on one in their home environment.
Full-time | Days Monday - Friday | Will be a part of a Holiday rotation.
This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the service area of where the patients live, is in the 11 County Twin Cities Metro area.
