Job summary

Interim HealthCare of the Twin Cities is seeking a Home Care Registered Nurse Case Manager to visit patients in their homes, perform head-to-toe assessments, and coordinate care with an interdisciplinary team. This full-time position offers independence and flexible scheduling in the Roseville, MN area.

The RN must hold an MN license, be willing to travel, and have CPR/BLS; two years of nursing experience and one year of case management experience are preferred.

Qualifications

• Graduate of an accredited school of nursing with MN RN license.
• Two years nursing experience preferred.
• One year case management experience preferred.
• CPR/BLS certification.
• Able to travel within a geographic area serviced by the office; valid driver’s license and proof of auto insurance required.

Responsibilities

• Perform head-to-toe assessments and nursing procedures.
• Visit home patients to evaluate care quality and review clinical notes.
• Participate in ongoing interdisciplinary patient assessment.
• Provide physician-ordered services per the plan of care.
• Counsel and educate patient and family on healthcare needs.
• Complete timely clinical notes and communicate with the care team.

The Home Care Registered Nurse (RN) Case Manager enjoys independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The RN Case Manager would provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management,ostomy education, infection prevention, and home safety.

Hours

Full-time | Days | Monday - Friday | Holiday rotation (work approximately two holidays per year) | Weekend rotation may apply dependent upon staffing levels.

Territory

This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position of where the patients live would be the southeast section of the Twin Cities Metro area.

A few must-haves for this position:

• Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
• Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.
• CPR/BLS Certification
• Two (2) years of current nursing experience preferred.
• One (1) year case management experience preferred.
• Preferred:Community health and a wide range of Medical-Surgical experience.

Some functions of what you would do as a RN Case Manager:

• Performassessments and procedures, which requireknowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
• Visit in home clients to evaluate the quality of care being provided and review clinical notes.
• Participate in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Counsels and educates the patient/client and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing the patient’s/client’s progress.
• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
