Job summary

CVS Health seeks an experienced Registered Nurse to perform in-home assessments and care planning for members in MLTSS and D-SNP/FIDE programs. This full-time field/remote role requires up to 75% travel across Essex and nearby counties in New Jersey, with standard 8:00 a.m.–5:00 p.m. hours.

The role emphasizes coordinating with providers, patients, and care teams, delivering high-quality care, and completing NJ Choice Certification to maintain eligibility for continued employment.

Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum of 3 years of clinical experience in hospitals, home health, or ambulatory care.
• Residency in Essex or adjacent counties; ability and willingness to travel up to 75% within Ocean and surrounding counties.
• Successful completion of NJ Choice Certification, including exam score 80% or higher, state training modules, and field mentoring as required.

Responsibilities

• Conduct face-to-face home visits for members enrolled in MLTSS and/or D-SNP/FIDE.
• Perform in-person assessments for non-MLTSS members to determine medical needs and referrals.
• Coordinate with providers, authorized representatives, and interdisciplinary care teams.
• Develop individualized care plans and authorize services within MLTSS/FIDE benefits.
• Document assessments and care planning activities in the EHR in a timely manner.
