Job summary

CVS Health Corporation is seeking a licensed RN for a full-time field/work-at-home role in New Jersey, with up to 75% travel across Essex and surrounding counties. Work hours are Monday–Friday, 8:00 a.m.–5:00 p.m.

EST. The role involves developing, implementing, and supporting health service strategies, coordinating with providers and interdisciplinary teams, and documenting assessments and care plans in the electronic health record.

Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum of 3 years of clinical experience.
• Residency in Essex or adjacent counties; willingness to travel up to 75% within Ocean and surrounding counties.
• Successful completion of NJ Choice Certification (80%+ exam, state training modules, field mentoring).

Responsibilities

• Conduct in-person home visits and assessments for MLTSS/D-SNP/FIDE members.
• Coordinate with providers and interdisciplinary care teams.
• Develop individualized care plans and authorize services within program benefits.
• Document assessments and care planning activities in the electronic health record.
• Mentor new staff after proficiency is achieved.
