Job summary

The Valley Health System is seeking a Registered Nurse to lead discharge planning and provide timely utilization management services to optimize patient outcomes and reimbursement.

Responsibilities include collaborating with patients, families and the healthcare team to ensure safe, individualized discharge planning and efficient care transitions across units. Requires RN licensure in Nevada and 3+ years of clinical experience, with preference for UR/UM or Case Management.

Qualifications

• Three years of clinical experience in varied settings.
• Two years of experience in Utilization Review / Utilization Management or Case Management preferred.
• Knowledge of social and physical factors that affect discharge and functional status.

Responsibilities

• Establish safe, individualized discharge plans and care coordination.
• Provide proficient timely utilization management to maximize reimbursement.
• Collaborate with patients, families and the healthcare team to ensure smooth transitions.
