Qualifications

• Minimum three years in varied clinical settings.
• Two years in Utilization Review, Utilization Management or Case Management preferred.
• Knowledge of social and physical factors affecting discharge.
• Knowledge of community resources to meet post-discharge needs.

Responsibilities

• Establish safe, individualized discharge planning.
• Provide timely utilization management to maximize reimbursement.
