Job summary

SSM Health’s Monroe clinic is seeking a Registered Nurse to coordinate discharge planning and ensure seamless transitions to the next level of care. The role involves collaboration with an interdisciplinary team and adherence to CoP standards for discharge planning.

The position requires at least three years of acute care experience, Wisconsin RN licensure (or compact state) and active engagement in care coordination and patient advocacy.

Qualifications

• Graduate of accredited school of nursing.
• RN license in Wisconsin or a compact state.
• Minimum three years of acute care nursing experience.

Responsibilities

• Ensure compliance with CoP discharge planning.
• Identify and escalate gaps related to inpatient and observation length of stay.
• Assess high-risk patients and develop discharge plans with the multidisciplinary team.
• Collaborate with team on opportunities for improvement in standard work.
• May assist with utilization management, ED case management, admission processes and liaison with post-acute providers.
• Apply evidence-based practice and ANA ethics to nursing care.
