SSM Health is seeking a Registered Nurse in Monroe, WI, to assess, plan, implement, and evaluate discharge planning needs in collaboration with an interdisciplinary team. You will ensure CoP compliance, assess readmission risks, and coordinate with multiple care settings to transition patients to the next level of care.

The role requires a nursing degree, RN license in relevant states, and at least three years of acute care experience. Day shift, regular full-time position in Case Management.

Qualificações

• Graduate of accredited school of nursing or licensing equivalency.
• Three years acute care experience.
• RN license in applicable states as required by work location.

Responsabilidades

• Ensures compliance with discharge planning CoP.
• Identifies gaps/barriers related to inpatient and observation length of stay.
• Assesses high-risk patients for readmission and collaborates on discharge plan with multidisciplinary team.
• Participates in standard work improvement with team.
• May handle utilization management, ED case management, admission processes, and liaison with post-acute providers.
