Qualifications

• Must hold a current RN license in the practicing state.
• Minimum 3 years of clinical nursing experience.
• BLS for Healthcare Providers certification.
• Valid driver's license and access to a vehicle.

Responsibilities

• Provide direct patient care and adjust nursing plans as needed.
• Perform initial assessments and develop care plans per physician orders.
• Complete documentation (visit assessment, 485, OASIS) accurately and timely.
• Communicate with physicians and care team; update on patient status.
• Participate in discharge planning and post-discharge coordination.
• Engage in chart audits and quality improvement activities.
• Maintain patient records in compliance with agency, state, and federal guidelines.
• Deliver age-appropriate care across a defined patient population.
