Job summary

UMass Memorial Health is seeking an experienced Registered Nurse for discharge planning and case management. You will assess patients, coordinate care across the continuum, and engage with payers to ensure coverage and timely information sharing.

The role emphasizes collaboration with physicians, care teams, and community resources to optimize patient outcomes and cost efficiency. A Massachusetts RN license is required.

Qualifications

• Massachusetts licensure as a registered nurse (MA RN).
• Three (3) years clinical experience in discharge planning or case management.
• Strong organizational and communications skills; ability to use specialized computer-based systems for data gathering, reporting, and analysis.

Responsibilities

• Assess patients’ physical, functional, and psychosocial needs using screening tools and medical records.
• Collaborate with care team to plan and coordinate discharge and post-discharge resources.
• Communicate with third-party payers to clarify benefits and ensure timely information exchange.
• Monitor and evaluate discharge plans, updating goals and timelines as needed.
• Support quality improvement initiatives and adherence to regulatory requirements.
