Job summary

SSM Health is seeking a Registered Nurse to manage discharge planning at WI Monroe Clinic Hospital. You will assess patients, collaborate with the interdisciplinary team, and ensure discharge plans meet CoP requirements.

The role requires three years of acute care experience and a nursing degree, with licensure in WI or a compact state. Expect daily responsibilities in case management and care coordination within the 32-hour work week.

Qualifications

• RN with nursing degree and WI or compact-state licensure.
• Three years acute care experience required.
• Strong discharge planning and care coordination skills.

Responsibilities

• Ensures CoP discharge planning compliance.
• Identifies gaps related to inpatient and observation length of stay.
• Assesses high-risk patients; develops and implements discharge plans with the multidisciplinary team.
• Participates in opportunities to improve standard work.
• May manage utilization, ED case management, admission processes and liaison with post-acute providers.
• Applies evidence-based practice to nursing care and age-specific patient needs.
• Upholds professional nursing standards per the ANA Code of Ethics.
• Maintains alertness and safety during duties.
• Performs other duties as assigned.
