Job summary

UnitedHealth Group is seeking an experienced Registered Nurse to support care management and transition planning in a hospital setting. The role focuses on coordinating care across the continuum, assessing patient needs, documenting outcomes, and ensuring safe, effective discharges.

Requirements include an Associate degree, an active RN license, and 3+ years in acute or direct care, plus ability to type and use Windows-based systems.

Qualifications

• Associates degree required.
• Current, unrestricted RN license in the state of employment.
• 3+ years of hospital/acute care or direct care experience.
• Must be able to type and navigate Windows-based environment.

Responsibilities

• Maintains knowledge of care management, care coordination changes, utilization review changes, and regulatory requirements.
• Assesses physical, psychosocial, and economic needs for transition of care planning.
• Documents and validates data to monitor interventions and outcomes.
• Interviews and collects data and chart reviews for readmission.
• Communicates with outpatient providers to enhance continuum of care.
• Integrates performance improvement and customer service principles.
• Adheres to policies and regulatory requirements.
• Leads coordination of patient-centered care and safe discharge planning.
