Job summary

Advocate Health seeks a registered nurse to manage caseloads, assess patients, and develop and implement care plans in Milwaukee. Qualifications

• Registered Nurse license in the practicing state.

Responsibilities

• Conducts initial assessment to obtain physical, social and environmental data.
• Establishes plan of care in accordance with physician orders and patient needs.
• Provides direct patient care and adjusts nursing care processes as needed.
• Completes documentation including visit assessment and orders accurately and promptly.
• Communicates with physicians to confirm and update patient plan of care.
• Facilitates discharge planning and continuity of care after discharge.
