Job summary

Optum is seeking a Case Manager RN in Wisconsin to coordinate patient services across the continuum of care, determine admission appropriateness, plan discharge, and collaborate with physicians, nurses, and social workers to ensure timely, high-quality care.

This on-site role requires an Associate’s Degree in Nursing, an active RN license, 1+ year of hospital and community resources experience, and strong communication skills. Schedule is Mon–Fri 8:00 a.m.–4:30 p.m. CST with every 6th weekend.

Qualifications

• Associate's Degree in Nursing and active RN license is required.
• 1+ years of experience with/knowledge of community resources, policies, and procedures.
• 1+ years of hospital and inpatient medical/surgical unit experience.
• 1+ years of clinical experience with/knowledge of age groups from newborn to geriatric.

Responsibilities

• Directs disposition of patients and utilization considerations.
• Leads interdisciplinary collaboration.
• Communicates with staff, physicians, patients, and families.
• Coordinates patient care processes to achieve outcomes and manages resource utilization.
• Educates patients and families about options within level of care.
