Job summary

Mission Hospital in Asheville, NC, seeks an experienced Case Manager Registered Nurse to support patients and families through the care journey. You will coordinate services, develop care plans, and partner with physicians, nurses and department leaders to ensure safe transitions and high-quality care across the continuum.

The role requires a nursing license, 2+ years in case management (or 3+ years clinical), and InterQual experience. A sign-on bonus is available for qualified candidates.

Qualifications

• 2+ years in Case Management or 3+ years in a clinical setting.
• InterQual experience preferred.
• RN licensure to practice in state(s) of? required by law and regulation.

Responsibilities

• Coordinate the plan of care and drive discharge planning with the multidisciplinary team.
• Assess psychosocial, medical and discharge needs and available resources.
• Serve as liaison between physicians, patients, families, hospital staff and external agencies.
