Mission Hospital in Asheville, North Carolina, seeks a Case Management Assistant to support discharge planning and post-acute placement. Qualifications

• Medical knowledge and terminology.
• Minimum of one year experience in a health care setting.

Responsabilités

• Coordinates with RN case manager and social worker for support; communicates updates on progress.
• Provides clerical support in multidisciplinary rounds and updates Tempo.
• Makes referrals through the web-based system to PAC providers with required medical records.
• Monitors PAC referrals and expedites discharge where possible.
• Prioritizes workload to ensure timely contact with PAC providers.
• Sends additional information to PAC providers when requested.
• Communicates status of PAC referrals, bed availability and barriers to resolve.
• Communicates discharge results with patient, including signing the Patient Choice letter.
• Delivers the Important Medicare Message (IMM) when applicable and manages Medicare discharge dates.
• Obtains physician signatures on Case Management documents.
• Sets up transportation and generates discharge packets for PAC transfers.
• Uses Case Management systems and maintains provider relationships.
• Demonstrates knowledge of regulatory requirements and company ethics policies.
