Job summary

Mission Hospital in Asheville, NC seeks an experienced Case Manager to support patients and families throughout the care journey. You will coordinate services, develop care plans, and partner with physicians, nurses, and leaders to ensure safe transitions and high-quality care.

You will identify at-risk patients during transitions, assess needs, and drive discharge planning with the multidisciplinary team to optimize outcomes. RN licensure and InterQual familiarity are required.

Qualifications

• RN licensure in the state of practice
• Experience coordinating care across inpatient and observation settings
• Discharge planning experience and collaboration with multidisciplinary teams
• InterQual utilization experience and adherence to criteria

Responsibilities

• Provide case management services for inpatient and observation patients as assigned.
• Identify patients at risk for adverse outcomes during transitions between care settings.
• Perform comprehensive psychosocial, medical, and discharge needs assessments.
• Coordinate the plan of care and drive the discharge plan with physicians and the care team.
• Partner with Social Services to ensure appropriate post-acute care.
• Evaluate progression using InterQual criteria and escalate issues as needed.
• Serve as liaison between physicians, patients, families, staff, and outside agencies
