Requirements include a BSN, current RN license, and at least 5 years of clinical experience; MSN is preferred and case management certification is highly recommended within 2 years of employment.

Qualifications

• Graduate from an accredited school of nursing. BSN required.
• MSN preferred. Minimum 5 years of relevant clinical experience.
• Previous Case Management experience preferred.

Responsibilities

• Identifies patients who would benefit from Case Management interventions based on an initial screening assessment.
• Assesses data and interviews patient/family to determine needs.
• Develops and implements a Plan of Care with nursing to improve outcomes and coordinate discharge planning.
• Monitors discharge plans and evaluates changes with the multidisciplinary team.
• Documents actions in the Electronic Medical Record and communicates with providers/payors.
