Houston Methodist The Woodlands Hospital is seeking a Case Manager (RN) to assess clinical conditions and plan for care transitions and discharge planning for a designated patient population. The role collaborates with physicians and the multidisciplinary team to ensure safe, quality patient care and appropriate utilization of inpatient resources.

The position requires Texas RN licensure, a strong background in hospital nursing, and experience in discharge planning and care coordination.

Qualifications

• Graduate of education program approved by the credentialing body for the required credential(s) indicated below in the Certifications, Licenses and Registrations section.
• Bachelor’s degree preferred

Responsabilités

• Communicates clinical information to payors and post-acute care providers to ensure safe transition and continuity of care.
• Discharge planning and coordination of care with the social worker and physicians to ensure medically necessary admission and continued stay.
• Facilitates discharge planning activities and throughputs, participates in multidisciplinary rounds, and supports safe, efficient patient care delivery.
