Job summary

Houston Methodist Willowbrook Hospital is seeking a Case Manager (CM) who is an RN to assess clinical condition and plan case management, including care transitions and discharge planning for a designated unit. Qualifications

• RN required with Texas licensure; must obtain permanent Texas license within 60 days
• Bachelor’s degree preferred
• Three years hospital nursing clinical experience
• Acute care case management experience preferred
• Knowledge of Medicare, Medicaid and managed care requirements
• Discharge planning, utilization management and case management knowledge

Responsibilities

• Communicates clinical information to payors and post-acute providers for safe transitions
• Assesses patients timely and reviews chart for medical necessity and throughput
• Facilitates discharge planning activities with the care team and family
• Participates in multidisciplinary rounds to address barriers to throughput and discharge
