Job summary

Houston Methodist seeks a Case Manager (CM) Certified to plan comprehensive case management, including care transitions and discharge planning for designated units. The role drives improvement, mentors staff, and collaborates with physicians and care teams to achieve quality outcomes.

Qualified candidates will have RN licensure in Texas (permanent within 60 days), a magnet-certified case management credential, and at least five years of hospital nursing with CM experience.

Qualifications

• RN license in Texas with permanent license within 60 days (if establishing residency).
• Magnet ANCC-recognized Case Management certification: ACHPN-HPCC/ CCM/ CMC/ ACM-NBCM or similar.
• Five years hospital nursing experience, including three years in case management.

Responsibilities

• Collaborates with physicians and the multidisciplinary team to coordinate care and transitions.
• Communicates with patients, families, and payers to ensure safe, timely discharge planning.
• Serves as a preceptor and educator for other case managers and social workers.
• Leads performance improvement and ensures compliance with CMS requirements.
