Job summary

Houston Methodist Willowbrook Hospital is seeking a Case Manager PRN (RN) to comprehensively plan for case management of a target patient population on designated unit(s). You will work with physicians and the health care team to ensure medically necessary admissions and efficient discharge planning, while maintaining high-quality patient care.

Responsibilities include collaboration with social work, discharge planning, and communicating clinical information to payors.

Qualifications

• Graduate of education program approved by the credentialing body for the required credential(s).
• RN license with Texas Board of Nursing; Texas licensure required within 60 days (if establishing Texas residency).
• Three years hospital clinical nursing experience, including two years in case management.

Responsibilities

• Collaborates with physician and interprofessional health care team to facilitate care for designated case load; monitors progress and ensures plan of care is patient-focused and cost-effective.
• Serves as a preceptor and coaches/mentors staff and students; acts as a resource for case management and social work resources.
• Performs review for medical necessity of admission, continued stay and resource use; applies utilization criteria to monitor appropriateness of care.
• Documents assessment and interventions; analyzes and reports on quality and patient satisfaction initiatives.
