Job summary

RN.com is seeking an RN Case Manager to coordinate patient care from admission through discharge in an acute hospital setting. You will collaborate with physicians, nurses, and external providers to remove barriers and support safe, efficient transitions.

Requirements include TX/compact RN license, BLS, and 1+ year of case management experience within a hospital context. 13-week assignment with 8:00 AM–4:30 PM shifts.

Qualifications

• TX/compact RN license required
• BLS certification required
• 1+ year case management experience in an acute hospital setting required
• Acute hospital experience within the past year with no more than 90-day break preferred

Responsibilities

• Coordinate patient care from admission through discharge.
