Job summary

Advocate Health in Wisconsin is seeking a Part-time RN Case Manager PRN to help patients transition safely through hospital stays. You will partner with patients, families, and the care team to assess needs, coordinate services, plan discharges, and support utilization management.

The role requires an RN license, a preferred Bachelor’s Degree in Nursing, and at least two years of clinical nursing experience.

Qualifications

• Conduct comprehensive nursing assessments and create timely care plans.
• Coordinate discharge planning with multidisciplinary team to meet goals.
• Provide continuity of care and discharge planning aligned with regulatory standards.
• Perform concurrent/ retrospective reviews using approved criteria.
• Collaborate with Social Work and support staff to implement safe care plans.

Responsibilities

• Ensure timely discharge planning and regulatory compliance.
