Job summary

Advocate Health is seeking an experienced Registered Nurse to manage caseloads in specialty areas, perform initial assessments, and establish comprehensive care plans aligned with physician orders.

The role requires direct patient care, ongoing evaluation of treatment responses, and timely documentation. You will collaborate with physicians, families, and the health care team to ensure safe discharge planning and continuity of care.

Qualifications

• Registered Nurse license in state of practice.
• BLS certification required within 6 months unless exempted.
• Valid driver's license and insured vehicle.

Responsibilities

• Manage caseloads in specialty areas and conduct initial assessments.
• Establish care plans per physician orders and patient needs.
• Provide direct patient care and adjust nursing plans as needed.
• Communicate with physicians and family; participate in discharge planning.
• Document visit assessments, orders, and communications accurately and timely.
• Participate in chart audits and quality improvement activities.
• Maintain patient records per agency and regulatory standards.
• Demonstrate age-appropriate care principles and growth/development considerations.
