Responsibilities

• Perform thorough patient assessments and develop care plans during discharge planning and care transitions.
• Aggregate, analyze, interpret and report data on patient outcomes and resource utilization.
• Provide continuity of care and discharge planning for clinically complex patients.
• Provide advanced clinical guidance and mentorship to frontline care management team.
• Advocate for patients and families within the healthcare system to optimize resources.
• Initiate internal and external referrals for timely care progression.
• Communicate with the healthcare team and coordinate multidisciplinary rounds to resolve discharge barriers.
• Lead discussions with physicians and treatment teams on best practices in patient care plans.
