Job summary

Atrium Health Wake Forest Baptist in Bermuda Run, NC seeks a part-time RN Case Manager to coordinate discharge planning and ensure high-quality patient care. You will assess needs, develop a Plan of Care with the care team, and communicate with providers and payors.

A BSN is required; MSN is preferred; current RN licensure in the state and at least 5 years of clinical experience are required. The role collaborates with physicians, nursing, and community services to move patients through the care

Qualifications

• Graduate from an accredited school of nursing. BSN required. MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Current RN licensure in the appropriate state; case management certification encouraged within 2 years.

Responsibilities

• Identifies patients for Case Management interventions based on discharge needs.
• Assesses data and interviews patient/family to evaluate needs.
• Develops and implements Plan of Care with nursing and team.
• Coordinates discharge planning with physicians, payors, and community providers.
• Monitors and modifies Plan of Care; documents actions in EMR.
• Participates in rounds to communicate discharge planning and patient progression.
• Identifies issues and suggests interventions to reduce duplication.
• Monitors medical necessity of admissions and level of care.
• Applies quality improvement principles to performance and practice.
• Provides age-appropriate patient and family planning and communication.
