Job summary

Advocate Health is seeking an experienced Case Manager (RN) to identify patients who will benefit from discharge planning and to collaborate with a multidisciplinary team to optimize outcomes. The role requires a BSN with MSN preferred and a current RN license in the applicable state.

Graduate from an accredited school of nursing.
BSN required; MSN preferred.
Minimum 5 years of relevant clinical experience.
Current RN licensure to practice in the applicable state; case management certification recommended within 2 years.

Responsibilities

Identify patients who would benefit from Case Management interventions.
Assess data by interviewing patients/families and evaluating needs.
Develop and implement Plan of Care with nursing to improve outcomes and manage costs.
Collaborate with multidisciplinary team and providers to execute the Plan of Care.
Coordinate discharge planning and monitor its effectiveness; adjust as needed.
Document actions in EMR and communicate with providers and payors.
Participate in rounds and drive patient progression through the care continuum.
