Journey Nursing Services in Washington state is seeking a Case Manager / RN Delegator to provide case management for Private Duty Nursing clients and RN Delegation services. The role combines leadership, care coordination, and hands-on nursing in community settings.

We offer full-time or part-time opportunities with flexible scheduling, allowing nurses to maintain bedside skills while building lasting patient relationships in home and community-based care.

Qualifications

• Current Washington State Registered Nurse (RN) license in good standing.
• Current BLS Certification.
• Minimum one year of nursing experience.
• Willingness to travel throughout the assigned service area.
• Excellent assessment, clinical judgment, and communication skills.

Responsabilités

• Manage an assigned caseload of Private Duty Nursing clients.
• Complete admission, renewal, and ongoing comprehensive nursing assessments.
• Develop, implement, and update individualized Plans of Care.
• Coordinate care with physicians, therapists, pharmacies, and other interdisciplinary team members.
• Complete initial and renewal RN Delegation assessments.
• Perform required RN Delegation supervisory visits and caregiver oversight.
• Provide education, competency validation, and ongoing support for caregivers.
• Deliver skilled nursing services and direct patient care as assigned.
• Provide skilled care for medically complex patients, including when applicable: tracheostomy care, ventilator management, enteral feeding, TPN, medication administration.
• Monitor patient progress and revise Plans of Care as clinical needs change.
• Maintain accurate and timely electronic medical record documentation.
• Participate in quality improvement, compliance, and regulatory initiatives.
• Foster a culture of teamwork, professionalism, accountability, and clinical excellence.
• Perform other duties as assigned.
