Job summary

Journey Nursing Services in Washington State seeks a Case Manager / RN Delegator to provide case management for Private Duty Nursing clients, complete comprehensive nursing assessments in patients’ homes, and coordinate care with physicians and the interdisciplinary team.

This role combines clinical leadership with hands-on nursing across private homes, assisted living, and family homes, offering a flexible full- or part-time schedule.

Qualifications

• Current Washington State RN license in good standing.
• Current BLS Certification.
• Minimum one year of nursing experience.
• Excellent assessment, clinical judgment, and critical thinking skills.
• Strong communication, organizational, and teaching abilities.
• Valid driver's license and willingness to travel throughout the assigned service area.

Responsibilities

• Manage an assigned caseload of Private Duty Nursing clients.
• Complete admissions, renewals, and ongoing nursing assessments.
• Develop, implement, and update individualized Plans of Care.
• Coordinate care with physicians and interdisciplinary team.
• Provide RN Delegation assessments and supervisory visits.
• Deliver skilled nursing services and provide direct patient care as needed.
• Maintain electronic medical record documentation.
• Participate in quality improvement, compliance, and regulatory initiatives.
• Foster teamwork and clinical excellence.
