Job summary

Journey Nursing Services is seeking compassionate Registered Nurses to provide case management, RN delegation, and skilled nursing care within Snohomish, King, Whatcom, and surrounding counties. The role combines leadership, education, and hands-on patient care in community settings.

You will work with Private Duty Nursing clients, collaborate with physicians and interdisciplinary teams, and support families and caregivers with comprehensive care planning.

Qualifications

• Current Washington State RN license in good standing.
• Current BLS Certification.
• Minimum one year of nursing experience.
• Strong communication, organizational, and teaching abilities.
• Valid driver’s license, reliable transportation, and willingness to travel throughout the assigned service area.

Responsibilities

• Manage an assigned caseload of Private Duty Nursing clients.
• Complete admission, renewal, and ongoing nursing assessments.
• Develop, implement, and update individualized Plans of Care.
• Coordinate care with physicians and interdisciplinary team members.
• Provide RN Delegation assessments and supervisory visits.
• Deliver skilled nursing services and direct patient care as assigned.
• Monitor patient progress and revise Plans of Care as needs change.
• Maintain accurate electronic medical record documentation.
• Participate in quality improvement and regulatory initiatives.
• Foster teamwork, professionalism and clinical excellence.
