Journey Nursing Services provides Private Duty Nursing, Home Caregiving, and RN Delegation across Washington state. This role offers case management for Private Duty Nursing clients, including comprehensive home assessments, plan of care development, and coordination with physicians and the interdisciplinary team.

Qualifications

• Current Washington State RN license in good standing.
• Current BLS Certification.
• Minimum one year of nursing experience.
• Valid driver’s license and willingness to travel within service area.

Responsibilities

• Manage an assigned caseload of Private Duty Nursing clients.
• Complete admission, renewal, and ongoing nursing assessments.
• Develop, implement, and update Plans of Care.
• Coordinate care with physicians and interdisciplinary team members.
• Perform RN Delegation assessments and supervisory visits.
• Provide caregiver education and ongoing support.
• Deliver skilled nursing services and direct patient care as assigned.
• Care for medically complex patients (e.g., tracheostomy, ventilation, TPN).
• Maintain timely electronic medical record documentation.
• Participate in quality improvement and regulatory initiatives.
