Job summary

Journey Nursing Services is seeking a dedicated RN to serve as Case Manager / RN Delegator in Washington. This full-time role involves assessing, coordinating, and overseeing Private Duty Nursing clients in their homes, with opportunities for Part-Time schedules.

Qualifications

• Current Washington State RN license in good standing.
• BLS certification required.
• Minimum of 1 year nursing experience in relevant setting.

Responsibilities

• Manage an assigned caseload of Private Duty Nursing clients.
• Perform RN delegation assessments and supervisory visits.
• Coordinate care with physicians, case managers, therapists, pharmacies, and other team members.
• Provide caregiver education, competency validation, and ongoing support.
• Deliver skilled nursing services and direct patient care as assigned, including clinically complex interventions.
• Maintain accurate and timely electronic medical record documentation.
• Participate in quality improvement, compliance, and regulatory initiatives.
