Job summary

Journey Nursing Services is seeking compassionate Registered Nurses to join our clinical team in Washington. The role combines case management with RN delegation, providing hands-on nursing care across home-based settings.

Qualifications

• Current Washington State RN license in good standing.
• Current BLS Certification.
• Minimum one year of nursing experience.
• Excellent assessment, clinical judgment, and critical thinking skills.
• Strong communication, organizational, and teaching abilities.
• Valid driver's license and willingness to travel throughout the assigned service area.

Responsibilities

• Manage an assigned caseload of Private Duty Nursing clients.
• Complete admission, renewal, and ongoing comprehensive nursing assessments.
• Develop, implement, and update individualized Plans of Care.
• Coordinate care with physicians, case managers, therapists, pharmacies, and other interdisciplinary team members.
• Complete initial and renewal RN Delegation assessments.
• Perform required RN Delegation supervisory visits and caregiver oversight.
• Provide education, competency validation, and ongoing support for caregivers.
• Deliver skilled nursing services and direct patient care as assigned.
• Provide skilled care for medically complex patients and related interventions.
• Monitor patient progress and revise Plans of Care as clinical needs change.
• Maintain accurate electronic medical record documentation.
• Participate in quality improvement, compliance, and regulatory initiatives.
• Foster a culture of teamwork, professionalism, accountability, and clinical excellence.
