Job summary

Journey Nursing Services provides Private Duty Nursing, home care, and RN delegation across Snohomish, King, Whatcom, and surrounding counties. This role includes case management for Private Duty Nursing clients, comprehensive home assessments, and care coordination with physicians and the interdisciplinary team.

The RN Delegation services, skilled nursing oversight, and hands-on care offer a dynamic, nurse-led environment with opportunities for professional growth and leadership.

Qualifications

Current Washington State RN license in good standing.
Current BLS certification.
Minimum one year of nursing experience.
Strong assessment, clinical judgment and teaching abilities.

Responsibilities

Manage caseload for Private Duty Nursing clients.
Conduct admissions and renewal nursing assessments.
Develop and update individualized Plans of Care.
Coordinate care with physicians and interdisciplinary team.
Perform RN Delegation assessments and supervisory visits.
Provide caregiver education and ongoing support.
Deliver skilled nursing care and direct patient care as assigned.
