Job summary

Journey Nursing Services in Washington State seeks compassionate RNs for a Case Manager / RN Delegator role with full-time or part-time options. This position combines case management, RN delegation, and direct patient care across Private Duty Nursing and home settings.

You will conduct comprehensive nursing assessments, develop and update plans of care, and coordinate with physicians and the interdisciplinary team. The role offers growth opportunities in a nurse-led, collaborative environment.

Qualifications

• Current Washington State RN license in good standing.
• Current BLS Certification.
• Excellent assessment, clinical judgment, and critical thinking skills.
• Strong communication and teaching abilities.
• Willingness to travel within the service area.

Responsibilities

• Manage caseload for Private Duty Nursing clients.
• Complete admission, renewal, and ongoing nursing assessments.
• Develop, implement, and update Plans of Care.
• Coordinate care with physicians, case managers, therapists, and pharmacies.
• Perform RN Delegation assessments and supervisory visits.
• Provide education and ongoing support for caregivers.
• Deliver skilled nursing services and direct patient care as assigned.
• Maintain timely EMR documentation.
