Job summary

Qualifications

• Bachelor's degree in a healthcare related field, social service field, or RN.
• Two years’ Healthcare/Hospital experience as RN or Social Worker preferred.
• Knowledge of basic computer skills and word processing.

Responsibilities

• Directs and coordinates the integration of Case Management into patient care, discharge and home planning processes.
• Coordinate Case Management communications with staff and external social service organizations.
• Develop and implement plans for discharge and post-hospital care with patients and families.
• Coordinate information and guidance to patients and families regarding post-discharge treatment and recovery.
• Represent the department on hospital committees and participate in program planning and evaluation.

Education

Bachelor's degree in healthcare related field or RN

Two years’ Healthcare/Hospital experience as RN or Social Worker
