You will partner with physicians, nurses, and department leaders to ensure safe transitions, clear communication, and high-quality care across the continuum. This role emphasizes compassionate, patient-centered service within our hospital network.

Qualifications

• RN required and Registered Nurse Diploma.
• 1 year experience required.

Responsibilities

• Guide patients and families through care journey and discharge planning.
• Coordinate education for patients, families, and caregivers.
• Develop individualized treatment and discharge plans with the care team.
• Complete psychosocial assessments and communicate findings clearly.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team.
• Promote values of compassion, respect, and excellence in all interactions.
