Job summary

Qualifications

• Bachelor's degree in healthcare-related field, social work, or RN.
• Two years hospital experience as RN or Social Worker preferred.
• Licensed as RN or Social Worker in state of employment.
• BLS within 30 days of employment and maintain for duration.

Responsibilities

• Directs integration of Case Management into patient care, discharge, and home planning with other hospital departments.
• Coordinate clinical care with physicians, nursing personnel, and other management team members for care planning.
• Complete psychosocial assessment on all new admissions within three business days.
• Develop and implement programs to assist patients and families with social and psychological factors.
