Job summary

Ntmconline is seeking a dedicated case manager to coordinate clinical care for patients, families and significant others, ensuring dignity and individuality in every interaction. You will complete psychosocial assessments within three business days, and work with physicians and nursing staff to plan patient care and discharge.

Responsibilities

• Demonstrates and respects the rights, dignity and individuality of every patient.
• Coordinates clinical care for patients, families, and significant others in illness-related planning.
• Completes psychosocial assessments on new admissions within three business days.
• Develops and implements programs to assist patients and families with social and psychological factors.
• Plans patient care with physicians, nursing staff, and management.
• Engages in individual or group intervention sessions related to care and adjustment.
• Arranges discharge and post-hospital care through community resources.
• Provides guidance in handling special patient cases or needs.
• Coordinates information and guidance to patients and families about post-discharge care.
• Refs to appropriate community and home health services for patients and families.
• Coordinates Case Management communications and liaises with external social service organizations.
• Participates in interdepartmental program planning and Case Management education.
• Represents the department on hospital committees.
• Develops Pastoral Care Program with Host hospital.
• Maintains harmonious relations with other departments and addresses social service issues.
