Job summary

Ascension is seeking a Registered Nurse to coordinate the interdisciplinary plan of care for the Case Management department from admission to discharge. You will integrate patient and family preferences, advocate for needed resources, and ensure appropriate level of care across the continuum.

You will monitor medical necessity, maintain documentation quality, collaborate with care transition teams and payers to optimize benefits and resources, and act as a liaison for post-discharge services

Qualifications

• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• BLS Provider preferred (American Heart Association or American Red Cross accepted).
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Responsibilities

• Creates plan for care across the continuum. Integrates patient/family preferences and values. Advocates for resources and removal of barriers for the patient.
• Monitors patients' medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of care plan.
• Serves as a resource for medical record documentation, level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care. Acts as a liaison between patient and post-acute services.
• Collaborates with payers for benefit optimization.
• Advocates for effective and efficient utilization of clinical resources.
