Job summary

Advocate Health seeks a registered nurse to manage caseloads in specialty areas, performing initial assessments and developing care plans aligned with physician orders. You will deliver direct patient care, adjust plans as needed, and document assessments, orders, and outcomes with attention to accuracy.

You will collaborate with physicians, family, and the health care team, participate in discharge planning, and engage in quality improvement activities while maintaining compliant patient

Qualifications

• Associate's Degree in Nursing required.
• Typically requires 3 years of experience in clinical nursing.
• Must have and maintain a valid driver's license and, if applicable, a vehicle with adequate insurance.

Responsibilities

• Carry caseloads of patients in specialty areas and perform initial assessments.
• Develop and implement care plans based on physician orders.
• Provide direct patient care and adjust nursing care as needed.
• Document visits, assessments, OASIS, 485, and verbal orders accurately.
• Communicate with physicians and care team to update plans.
• Participate in discharge planning and continuity of care.
• Participate in quality improvement and chart audits.
• Maintain accurate clinical and patient records.
